Intake

Prior to accessing services through a local provider, an individual must go through an eligibility determination process.

The eligibility process and service eligibility decision is done at the state level with the South Carolina of Disabilities and Special Needs (SCDDSN). Please call their intake number at 1-800-289-7012 or www.ddsn.sc.gov/apply to begin the eligibility determination process for individuals over the age of 36 months.

Once service eligibility is determined, a choice of local Case Management/Early Intervention providers is given. MaxAbilities is a great choice to meet your local Case Management needs. 

For individuals 36 months or younger, please contact BabyNet (DHHS) toll- free at 1-877-621-0865 to schedule a screening for your child. The MaxAbilities Early Intervention program - ENCOURAGING STEPS - is an option for Early Intervention as well as Case Management services for eligible individuals up to the age of 6.
